Terms of Agreement for Providers

Agreement and adherence to the following Terms of Agreement are required of all community providers (provider) who wish to submit and maintain an active listing with the Community Provider Database (CPD) – a free community service provided by The Ohio State University (OSU) intended to streamline the process whereby students seeking mental health care services in the community can be matched to a provider who meets their needs.

Agreement to these terms is indicated by logging into your account, checking the “I Agree” box at the top of this screen, and clicking “Continue”. OSU reserve the right to change these terms at any time and thereafter require providers to re-submit their agreement to any such new terms within a specified time period. Agreement is required for the use of this service.

At the current time, the CPD is only accepting listings from providers in or near communities surrounding The Ohio State University campuses. Providers must have an appropriate practice-related degree in the field of mental health care along with a matching license to practice in the State of Ohio. Any submissions that do not match these criteria will be discarded without notice.

All submitted listings, and any subsequent changes made by the provider, must first be approved by a CCS staff member before they become available on the CPD search. Approvals will typically occur during regular business hours – but are not guaranteed to occur within any time-frame. The approval process will include, but may not be limited to, verification of your practice and the current validity of your Ohio license. Approvals do not equate to an endorsement for the provider. We reserve the right to request additional documentation from any provider or practice at any time for purposes of our verification process. We also reserve the right to remove from the CPD any provider or practice who fails to provide such requested information in a timely manner. Removals from the CPD in general are at the discretion of CCS. Providers who do not update their information when asked will automatically be removed with notice.

Providers agree that all information submitted by them to be incorporated into their CPD listing will be accurate, current, and verifiable. Providers agree to maintain the accuracy of their listing by updating their practice information with the then most currently available information at least twice per year. Providers or practices whose listing is found to contain inaccurate or out-of-date information will be removed from the CPD and notified. Providers or practices who do not log-in within one year will be automatically removed from the CPD.

The CPD is provided as a free community service to mental health care providers in or near the campuses of The Ohio State University and potential clients seeking services in these areas. This service may be discontinued, limited, or modified at any time at the exclusive discretion of OSU. OSU reserve exclusive and final rights to decide whether or not a provider or practice shall be listed, and how they shall be listed, in the CPD.

Providers hereby agree to indemnify and release OSU and all of their respective agents, servants, representatives and employees from any and all causes of action, claims or demands of whatsoever kind, arising from or in any way connected with the CPD.

Provider application approval and removal are at the discretion of CCS. Approval of a provider does not equate to an endorsement for the provider.
